Insurance
PT Works accepts most insurances such as PPO plans, Workers Compensation plans, motor vehicle insurance, and we are certified as a Medicare provider. For further information regarding your specific plan of coverage you may call us at
650-947-9646. For your convenience, we verify the insurance benefits for all of our patients prior to the first visit. We want to help you understand your financial responsibility before you begin your treatment, however, we encourage you to confirm your insurance coverage as the information we receive is not a guarantee of payment.
Accessing Physical Therapy
Under California law, you have direct access to physical therapy services for a period of up to 45 calendar days or 12 visits, whichever occurs first. After the period of time or number of visits are exhausted and services are still necessary, a physical therapist may continue providing physical therapy only after receiving a signed and dated referral from a physician holding a license from the Medical, Osteopathic or Podiatric Boards of Medicine. The physician must indicate approval of the physical therapy plan of care and that an in-person examination was conducted by the physician. Patients have direct access, however, this may not be acceptable to your insurance who may require a referral for medical necessity and/or prior authorization to process your claims. What to Expect
Please arrive 15 minutes prior to your first appointment for registration. On your first visit, your therapist will evaluate you to determine a treatment plan based on your identified issues. Therapists will provide treatment after the evaluation and begin to develop a personal exercise program. Finally, the therapist will end of the sessions with a modality after the first 45 minutes. Appointments typically run 60-90 minutes.
Children Under 18
Registration forms must be signed by a parent or guardian. If you are unable to attend your child's first appointment, please speak to our office prior to your child's visit with the therapist.

Functional Indices - Filling out the applicable form will help your physical therapist establish goals and a baseline of your pain and functional ability for specific activities for daily living.
